Patients from outside the UK must complete an application HSCR1 .You need to provide documentary evidence (i.e. passport and visa) which staff at the Surgery will copy and return the original documents to you. We unfortunately cannot accept your application unless you can provide these documents. Please fully complete your applications or we will be unable to accept it.
Once approval is given by the Doctor your application will be forwarded to the Health Authority for checking and approval. If the Health Authority approves your status and eligibility for NHS services you will be sent a Medical Card to the address you provided on your application. They will notify the Surgery who will set up your clinical record.
All new patients will also need to complete a new patient health questionnaire giving details of your important past medical history, medication you are on, allergies etc.
All new patients are offered a health check with a doctor to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
