Patients registering with the Practice will be offered a New Patient Appointment with the Pharmacist, Practice Nurse, or Doctor.

If Patients fail to attend for this appointment you may not be accepted onto the Practice List. Only in exceptional circumstances will a further appointment be arranged.


All new patients will also need to complete a new patient health questionnaire giving details of your important past medical history, medication you are on, allergies etc.

All new patients are offered a health check with a doctor or our pharmacist to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. 

Please note this appointment is primarily to collate your medical history and it may be appropriate to attend again thereafter with a Doctor to follow up on medical concerns.

Please bring any medications you are taking to the new patient medical, so the doctor or pharmacist can see them. If you have any relevant past medical notes, or vaccination history, then please bring them.
