Registering if You Are From Outside Northern Ireland

 

What form you need

You must complete the HSCR1 registration form to register if you are from outside Northern Ireland.
This is the correct form for people coming from Great Britain, the EU, or overseas. 


What documents you will need

To register, you must show that:

a) You are lawfully in Northern Ireland
(for example: valid visa, settled/pre-settled status, UK or Irish passport)
b) You are living here
(for example: official proof of address such as a utility bill, tenancy agreement)
c) You have a reason for being here
(e.g., employment payslip, benefit letter)

What else you must do

You also need to complete a New Patient Health Questionnaire.
This asks about:

  • Any long-term illness

  • Medications you take

  • Allergies

  • Past health conditions 

When your HSCR1 is complete and we have your correct documentation, this will be sent as an electronic form via text


What happens after you submit your forms

Once your forms and documents are checked:

  • You will be offered a new patient consultation with a doctor or practice pharmacist.

  • This appointment is to record your medical history and to confirm any repeat medicines you need.


How to get the forms

You can:

  • Pick up the form at reception

  • Download it (see link above) and fill it in at home

  • Complete it electronically on a computer or tablet (some browsers/devices may require you to save the file first)
