New Patient Registration
Information for new patients
If you live within our practice area, you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill, or other documents as noted. Please bring your medical card and photographic identification to reception.

If at all possible, we would kindly ask that these forms are submitted between 2pm and 4pm at our Donegall Road site.
Once your documentation is processed, a "New Patient" consultation can be offered, to establish your needs, so that we can provide you with the best possible level of care.
We would kindly ask that to help reduce crowding, and to avoid delays to your registration, that you carefully complete these as fully as you can at home, before attending the surgery.
Registration Forms
To register as an individual or for everyone in your household, you will need to complete a
- Registration form (HS200, or HSCR1 form, depending on your circumstances)
- Health Questionnaire (which will provide useful information whilst we wait for your medical records to arrive from your previous doctor
All new patients are offered a health check with a doctor, nurse, or our pharmacist to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Whilst your registration is in process you can still access services if you have an immediately necessary, urgent need that cannot wait until after your new patient appointment.
Please note the new patient appointment is primarily to collate your medical history and it may be appropriate to attend again thereafter with a Doctor to follow up on medical concerns.
We also need new patients to carefully read and understand our New Patient's Medicine policy; in the interests of safe and effective prescribing, this gives an expection of how we manage prescribing, and our patient's responsibilities with regards to managing their medication
Medical treatment is available from the date of registration. Please contact reception for further information.
Appointments for New Patients
Patients registering with the Practice will be offered a New Patient Appointment with the Pharmacist or Practice Nurse.
If patients fail to attend for this appointment, once arranged, only in exceptional circumstances will a further appointment be arranged.
All new patients will also need to complete a new patient health questionnaire giving details of your important past medical history, medication you are on, allergies etc.

Please note this appointment is primarily to collate your medical history - it is not generally with a Doctor - and it may be appropriate to attend again thereafter with a Doctor to follow up on medical concerns.
Please bring any medications you are taking to the new patient medical, so the doctor or pharmacist can see them. If you have any relevant past medical notes, or vaccination history, then please bring them.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
Kensington Medical Centre
There is wheelchair access to the Kensington Medical Centre from the car park.
The surgery is a modern building with disabled access to all rooms on the ground floor.
A toilet suitable for wheelchair access is situated on the ground floor.
Finaghy Health Centre
Disabled car park spaces are close to the main entrance.
The consultation and treatment rooms are on the ground floor of the building and fully accessible to wheelchairs.
