Moving Your GP Practice
If you are registered with another GP surgery in Northern Ireland and wish to move your registration to Kensington Group Practice, please follow the steps below.
You must complete the HS200 form to transfer your registration. This also applies if you are moving from a GP surgery elsewhere in the UK (for example, England, Scotland or Wales).
How to Complete Your Registration
1. Get the HS200 form
You can:

Collect it from reception and complete it by hand, or

Download and complete it at home, including editing it on your computer (if supported by your device).
2. Provide Identification
When you bring your completed form to the surgery, please bring:

Photographic ID (e.g. passport or driving licence)

Proof of address in Northern Ireland
We will copy your documents and return the originals to you.
What Happens Next
Once you submit your completed HS200 form with the required ID and proof of address:

We will check your documents and add you to our patient list.

Your medical records will be transferred from your previous GP practice under standard NHS procedures.

We will contact you if we need any further information.

If your registration is not processed immediately, it’s because we need to verify your details. We will let you know if anything is missing.
If You are Moving From Outside Northern Ireland
If you have lived outside Northern Ireland (e.g. in Great Britain or overseas) and have not previously been registered with a GP in Northern Ireland, you may need a different registration form (HSCR1) — please see our separate guidance on that.
